Terms and Conditions:
Reservation Confirmation: Submitting this form does not guarantee your reservation. A confirmation email will be sent to the provided email address within 24 hours to confirm your booking.
Payment: Payment of $199 is due upon confirmation of your reservation. Payment instructions will be included in the confirmation email.
Cancellation Policy: Cancellations made more than 48 hours prior to the scheduled session will receive a full refund. Cancellations made within 48 hours of the scheduled session will not be eligible for a refund. A Credit will be provided by LK Supreme Media towards future use.
Rescheduling: Requests for rescheduling must be made at least 24 hours prior to the scheduled session. Rescheduling is subject to availability.
Participants: The session fee covers up to 6 participants, including Mom. Additional participants may be subject to an additional fee.
Digital Images: The package includes 5 digitally edited selections. Additional digital images may be available for purchase at an extra cost.
Prints: The complimentary 8.5x11 print and any additional prints selected from the ala carte print gallery will be available for pickup at LK Supreme Studios within [specify timeframe].
Usage Rights: By participating in the session, you grant LK Supreme Studios the right to use the images for promotional purposes, including but not limited to social media, website, and marketing materials.
Liability: LK Supreme Studios is not liable for any injuries or damages that may occur during the session. Participants are responsible for their own safety and well-being.
Acceptance: By submitting this form, you acknowledge that you have read and agree to the terms and conditions outlined above.
